Oh dear. I do get vexed, when I see the standard advice for women, rolled out, because it results in 2/3rds of british women wearing bras of the wrong size; what hope for a T-girl!
The band size is based on the unmeasurable; the size of your chest, less mammary tissue. As women have a tapered rib cage, the fomula of adding 4 or5 inches came about as an attempt to get that measurement and it works for 1 in 3- great for the rest! Guess what, we are not all the same shape. A skilled bra fitter does it by eye, so if a tape measure is used, you are quite likely to end up with a pup.
For a T-girl, I advise using your under bust measurement as a starting point, unless you are blessed with a tapering rib cage.
If your boobs are 3 to 4 inches larger, you are in B cup territory but remember that cup size is not a constant. The amount of mammary tissue a cup is designed to contain, increases with band size, so a 36 B contains a similar amount of tissue, as a 38A.
Then there is the vexed question of cup size/fit. I bought a pair of bras 2 months ago. They were  a black and a white, apparently identical. The black was a great fit but [as a growing girl] the white is, only now, usasble. In the meantime, I have had to send a bra to the charity shop as it was too tight accross the top and was giving me the dreaded 4 boob look.
Moral, cups and boobs vary in size and shape.
Next problem: a quick try on will only tell you if a bra could be right for you. Bras very quickly change, with use, as the fabric relaxes. Do not buy one that is great on the last row of hooks; you are heading for the dreaded strap slipage, as the band relaxes and gets bigger. NO NO NO adjusting the sraps does not cure that, it means that the band size is too big. The strap adjustement is to control the fit accross the top of the bra and uplift. Also be aware that bras warm up. A bra that has a crease or two, when first put on, can look perfect after 5 minutes of wear. Lastly, only time will tell you if a bra is comfortable. I recently put on a new bra, which was a great fit and I was a happy girl, for 5 hours, at 6 I knew it was another one for the charity shop, as the eyes of the straps were rubbing me in the wrong place.
Next, ignore standard  advice about underwires chaffing; I am a 39 but 40 bras leave me with weals on the side of my chest; 38, no problem.
Finally [ a word, my dear readers are hugely glad to hear] consider shape, no pair of knockers are the same and your bra must be right for you, but  may not fit your sister. It is essential to get a bra that suits your boobs. Mine come from umder my armpits, but leave no cleavage; I need a bra that comes in at the sides and defines my boobs, a balconnete is a no no.

I use the under bust measurement as the band size and the over the cup measurement as the cup size and go from there.

the way to a good fitting bra for a man is to be professionally fitted at least every 6months i had a problem with my bras untill i called and spoke to a young lady who encouraged me to come in and let her properly fit me ,the difference in comfort was instant and she was very helpful no problems with me being a man with large breast

Sfem, thanks for the link to your interesting post; very helpful.  My chest size is 40" and I measure 43" across the fullest part of my bust.  Each breast measures just over 8".  That would make me a 40AA, but with a nice stretchy band, it looks like I might be able to wear a 38A.
